Job Summary
Community Behavioral Health Worker provides in-home and community-based support to older adults in Franklin County and the North Quabbin area. Responsibilities include conducting home visits to assess strengths and needs, identifying formal and informal support systems, creating and implementing care plans with clients and caregivers, coordinating services, providing coaching toward sustainable wellness, assisting with applications and bridging referrals, and maintaining documentation. The role requires strong communication, planning, and interpersonal skills; proficiency with Google Suite and Microsoft 365; ability to work in a fast-paced environment; and willingness to travel to clients’ homes. Starting pay range is $22.25-$24.72 per hour, with a $1,500 sign-on bonus.
Required Qualifications
- Bachelor's degree in social work or related field
- two years experience working with older adults or two years of home-based casework with individuals with mental health/behavioral health challenges preferred
