Job Summary

Engage individuals and families in home and community settings to identify strengths, barriers, and goals related to behavioral health and daily functioning. Provide skill building, coaching, and practical supports to help clients develop routines, coping strategies, and healthy communication patterns. Assist clients in navigating community resources while maintaining consistent communication with caregivers and internal teams. Deliver care using trauma-informed, strengths-based approaches and utilize databases and Electronic Health Record systems for accurate client documentation. Collaborate with internal programs and external partners to ensure quality of care while adhering to HIPAA, mandated reporting requirements, and professional ethical guidelines. Travel within designated service areas to provide services in homes, schools, and community settings.

Required Qualifications

  • A high School diploma or GED equivalent
  • A bachelor's degree in related field or minimum 3 years of direct relevant experience
  • Experience with multi-need individuals and families
  • Broad knowledge of community service systems
  • Advanced communication skills, both oral and written
  • Advanced organizational skills
  • Ability to bill Medicaid without restrictions
  • A valid driver's license and the ability to operate a motor vehicle
  • An appropriate level of auto insurance coverage
  • Ability to use a personal vehicle or company vehicle to provide services in homes, schools, and community settings
  • Ability to maintain confidential information
  • Ability to manage deadlines within a fast-paced, high-volume environment
  • Ability to operate in an Internet-based, automated office environment
  • Ability to maintain high-speed internet connection
  • Prolonged periods of sitting at a desk and working on a computer
  • Must be able to lift up to 15 pounds at times
  • Travel within the designated service areas

Desired Qualifications

  • Experience partnering with local behavioral health systems, housing, legal systems, and social services
  • 3+ years of experience in related field
