Job Summary
Assess strengths and needs of referred individuals, families, and communities while providing practical home and community behavioral health services. Meet clients wherever they are to support self-management of health and well-being, and carry out utilization review and quality assurance activities. Maintain necessary documentation, participate in program evaluation and team planning meetings, and acquire knowledge of community resources. Meet billing productivity requirements and support growth in neighboring counties. Requires three years of directly relevant experience or a Bachelor's degree, a high school diploma, and licensure. Licensed and able to operate a motor vehicle with appropriate insurance.

- Demonstrated a high degree of cultural awareness
- Experience with multi-need individuals and families
- Broad knowledge of community service systems
- Licensed and able to operate a motor vehicle with an appropriate level of insurance coverage
- Willing to participate in and lead cross-systems team-building activities
- Able to effectively communicate through verbal/written expression
- Must be able to operate in an Internet-based, automated office environment
