Community Behavioral Health Liaison I

Job Summary
Coordinate care between clients, providers, and community resources to ensure coordinated, person-centered behavioral health services in Saint Louis. Assess needs related to behavioral health, social services, and community supports; facilitate referrals to medical, mental health, housing, and social supports; develop and support individualized service plans with interdisciplinary teams; advocate to reduce barriers to care and improve access; build and maintain relationships with community partners; monitor client progress and follow up to ensure continuity of care; maintain accurate documentation in line with organizational and regulatory standards; participate in team meetings, case reviews, and program development activities; support outreach and community engagement initiatives; promote a recovery-focused, trauma-informed, and culturally responsive approach to services.
Required Qualifications
- Bachelor’s degree in social work, psychology, counseling, or a related human services field preferred
- Relevant experience in behavioral health, case management, or community services may be considered in lieu of education
- Must have a valid driver’s license with an acceptable driving record and current auto insurance
- Strong communication and relationship-building skills
- Ability to work effectively with diverse populations and maintain confidentiality
- Strong organizational and time-management skills
- Ability to multi-task and respond effectively in a fast-paced environment
- Ability to collaborate with interdisciplinary teams and community partners
