Job Summary

Provide individual, group, and family mental health services, case management, transportation, and advocacy to youth, foster parents, and community partners. Coordinate and monitor services across medical, educational, and social domains while regularly reviewing and modifying youth-based plans to ensure consistent progress. Establish strong relationships with families of origin to assist in reunification and stabilization efforts, delivering safety assessments and crisis response as required. Complete all documentation timely, participate in weekly supervision, and engage in continuous quality improvement activities. Travel daily to deliver community-based services in compliance with organizational and regulatory requirements.

Required Qualifications

  • Master's Degree in Social Work or comparable Human Services field from an accredited institution
  • A valid advanced professional license as required and acceptable by state or contract regulations
  • Must have a valid driver's license
  • Reliable transportation
  • Automotive insurance
  • Good driving record
  • Willingness to work flexible and non-traditional hours
  • Capacity to be flexible and responsive to youth served, foster caregivers, system partners, and internal customers
  • Capacity to remain objective and professional in all areas of job function
  • Proficient use of desktop and laptop computers, smart phones and tablets, printers, fax machines and photocopiers as well as software including word processing, spreadsheet and database programs

Desired Qualifications

  • 2 years of work experience working in direct service with youth and families
