Community Based Therapist - Hybrid

Community Based Therapist position offering hybrid work (title notes Hybrid, description implies in-person/home-based responsibilities). Responsibilities include performing comprehensive diagnostic assessments, providing patient visits, ensuring up-to-date treatment plans, completing progress notes, communicating about clients’ status, and collaborating with case management and psychiatric services. Requires a Bachelor's degree in Social Work or related field with one year of therapy experience or a Master's degree in Social Work or Professional Counseling, plus a Social Work/Professional Counseling license or equivalent (LSW, LISW, LISW-S, LPC, LPCC, LPCC-S, LMFT).
