Community Based Therapist- Fairfield County

Job Summary
Provide diagnostic assessments, treatment planning, and therapy to a diverse caseload of adults, adolescents, and families via telehealth, outpatient offices, or community-based settings. Develop and maintain therapeutic relationships while responding to mental health emergencies in a culturally responsive manner. Identify crisis interventions, access community resources, and coordinate referrals as a liaison for clients. Document all clinical interactions, progress notes, and legal/financial details accurately within agency guidelines. Participate in regular supervision, treatment conferences, and team meetings. Maintain required licenses and complete pre-employment screenings including background checks and drug tests.
Required Qualifications
- Master's Degree from relevant accredited program
- Bachelor's degree from relevant accredited program with 2 years' experience in social work or counseling
- Bachelor's Degree in a behavioral science or nursing
- LSW or LCDC III, LPC, MFT license
- Mission driven

- Ability to customarily and regularly exercise discretion and good professional judgment
- Flexible; able to work days, evenings, and/or weekends
- Strong organizational skills
- Ability to maintain relevant, timely documentation as required
- Effective computer literacy skills, including Windows applications, Microsoft Office Suite, and electronic health records
- Must be able to access work locations, offices, and homes as necessary, which may include ascending/descending stairs
- Possess a valid Ohio Driver's License with a safe driving record and valid insurance
- Successfully complete pre-employment screenings, including drug tests, background checks, and any other required assessments, such as a physical examination and TB test
