Community-Based Care Coordinator - Intensive Adult Support

Job Summary
Provide skills training and development to clients in group and individual settings guided by medical necessity. Conduct client assessments and formulate, develop, and revise treatment plans in collaboration with the treatment team. Advocate for clients to ensure access to needed services by establishing community relationships and resources. Manage crisis situations including emergency interventions and maintain a safe environment through ongoing milieu assessment. Report progress, barriers, and care changes to the team while completing timely documentation and billing procedures. Provide case management by working with families and community supports to assist clients in meeting goals.
Required Qualifications
- Bachelor's degree
- If incumbent holds a bachelor's of social work degree, incumbent must also possess a current, unencumbered Licensed Bachelor's Social Worker license in the State of Indiana
- Valid Indiana driver's license
- driving record that meets Eskenazi Health's driving policy requirements
- Basic knowledge of behavioral health
- Basic knowledge of substance use disorder and/or co-occurring disorders
- Basic knowledge of medical terminology
- Knowledge of client rights and responsibilities
- Must meet all population-specific competencies for role and service area
- Basic computer skills
- Crisis intervention skills
- Strong interpersonal skills
- Ability to teach a skill based upon person-centered learning needs and learning styles
- Ability to be both clear and concise in written and verbal communications
- Ability to work cooperatively in a multidisciplinary team
- Ability to think critically and problem-solve efficiently
- Ability to provide hands-on client care
- Ability to perform safety interventions including physical safety interventions required for service area
- Ability to lift a minimum of 15 lbs
