Job Summary
Community Associate for Family Partnerships supports NYCHA residents by coordinating social services, performing home visits, pre-screening residents for referrals, maintaining case records in the FPCMS database, and developing collaborative partnerships with NYCHA staff and community-based organizations to ensure effective referrals and outcomes. Responsibilities include direct referrals, data entry, documentation, and following up on referrals; familiarity with case management, state/city program referrals, and maintaining thorough documentation. Requires high school graduation or equivalent with at least three years of related community work experience; NYCHA benefits provided and opportunities to connect residents to financial empowerment, youth, senior, and educational programs. Remote work is not indicated for this posting; in-person work in Brooklyn, NY is expected.
Required Qualifications
