Job Summary
Provide community access services to adults with developmental or intellectual disabilities, conducting one-to-one support in integrated community settings. Assess clients’ skills and interests; develop and implement personalized Community Access Plans (CAP); connect participants with community resources and activities; facilitate participation in typical neighborhood activities; foster connections with non-disabled individuals; document services in the Medicaid Waiver Management Application (MWMA); coordinate with businesses and funders to identify resources; accompany clients to activities when needed; ensure client safety. Requires paid training and certification, CPR/First Aid, valid driver’s license with insurance, weekend/holiday availability, and compliance with DDID training requirements. Required Qualifications
- Must be 18 years of age
- Must have a high school diploma or its equivalent
- Must have at least one year of experience working with adults with developmental and/or intellectual disabilities
- Must complete all required training as outlined by DDID regulations, including first aid and CPR certification
- Must be able to read, understand and implement written and oral instructions
- Must pass a criminal background check
- Must pass pre-employment and random drug testing
- Must have a valid driver's license, good driving record, and proof of auto insurance
- Must be available to work weekends and holidays
