Job Summary
The Case Manager delivers holistic case management services to members at assigned site(s), including intake, assessment, goal setting, service planning, progress monitoring, advocacy, and referrals, while ensuring administrative compliance and accurate data/reporting. The role serves youth ages 12-18 in the Fort Worth Comin’ Up Gang Intervention Program, focusing on needs-based services to reduce gang violence. It entails documenting services/contacts, participating in a multi-disciplinary team, coordinating programming, maintaining safety policies, transporting youth as needed, and maintaining regular attendance. Requires a Bachelor’s degree (or equivalent experience), minimum year of youth-facing case-management and related experience, CPR/First Aid certification, and a valid driver’s license; bilingual ability and prior Boys & Girls Club program knowledge are assets.
Required Qualifications
- Bachelor’s Degree from an accredited college or university, or equivalent experience
- Minimum of one year of experience working with at-risk youth and/or gang-involved youth
- Minimum of 6 months of case management-related experience
- Ability to work regular program hours, which include day and evening, with occasional weekends
- Ability to assist youth in progressing and achieving their individual goals
- Ability to establish rapport and build relationships
- Ability to persuade, influence, and motivate others into action
- Ability to identify needs and find solutions
- Ability to work with at-risk youth
- Knowledge of case management techniques and/or processes
- Knowledge of conflict resolution or mediation techniques
- Knowledge of problems and barriers at-risk youth face
- Knowledge of technology and software, such as tablets, social media, cloud-based tools, and virtual meeting tools
- Must be a team player, willing to collaborate and make team decisions
- Must be able to obtain CPR and First Aid certification
- Must be able to pass agency background screens and drug screens at any time
- Must have reliable transportation, liability insurance, a satisfactory driving record, and a valid driver’s license for the operation of a motor vehicle relevant to carrying out duties

- Bilingual
- Work experience with or program knowledge of the Boys & Girls Club Movement (national and/or local Club) is an advantage
