Job Summary
Collaborative Care Mental Health Clinician (CCMHC) provides direct clinical care as part of the patient’s medical and mental health care team, serving as the cornerstone of the team with a pediatric primary care focus in Burlington/Williston, VT. Responsibilities include triage, clinical assessment, diagnosis with DSM/ICD-10, psychoeducation to patients and primary care providers, care coordination, case management, direct evidence-based psychotherapy (CBT/DBT), treatment planning and updates, crisis coordination, relapse prevention, and oversight of a registry for monitoring risk scores and program outcomes. Requires Master’s degree and VT licensure (LCSW or LCMHC); preference for 3+ years of post-master’s clinical experience and experience with pediatric populations and integrated behavioral health. Offered benefits package and a full-time schedule (40 hours/week) with daytime hours.
