Job Summary
Assess patient symptoms and create customized treatment plans using PHQ9, GAD7, and suicide risk tools. Monitor clinical changes and treatment side effects while supporting psychotropic medication management prescribed by primary care providers. Facilitate care plan adjustments for non-improving patients and manage behavioral health crises within the primary care setting. Provide flexible services through scheduled appointments and immediate warm handoffs during medical visits. Work within a multidisciplinary team to promote symptom reduction and optimal patient outcomes in a collaborative care model.
Required Qualifications
- Master's degree from an accredited institution in Social Work, Counseling, Marriage & Family Therapy, or related field
- A Mental Health Professional as defined within HB 1724
- Licensed Marriage & Family Therapist (LMFT)
- Licensed Mental Health Counselor (LMHC)
- SW Independent Clinical Lic (LICSW)
- Minimum two (2) years of experience in a behavioral health setting
- The primary location of this position will be Puyallup, Kent, or Auburn
- Excellent communication skills
