Job Summary
Collaborating Physician works with an APN(s) performing in-home and/or telehealth wellness assessments for patients designated by HarmonyCares Medical Group. Be available via telecommunication or in-person for consultation with APN to address patient assessment questions, regularly perform annual chart reviews, ensure adherence to policies, procedures, state laws and quality, and be accountable for outstanding customer service to all external and internal customers. Qualifications include an active/unrestricted medical license, board certification, residency completion, and experience in healthcare management with risk adjustment. Must reside in South Carolina. Office/remote work options supported by telehealth and in-home care workflows.
Required Qualifications
