Job Summary
Cognitive Behavioral Specialist responsible for managing a caseload of patients with cognitive behaviors, conducting visits at facilities and patients’ homes within designated service areas (Hilton Head Island area; Bluffton/Okatie area), delivering structured behavioral interventions, supporting caregivers, and coordinating with primary/ specialty providers to align behavioral and medical treatment plans. Key duties include conducting regular patient visits (initially weekly, then biweekly/monthly as needed), observing behavioral patterns, implementing structured behavioral protocols, monitoring changes in behavior and cognition for care plan adjustments, providing caregiver coaching and education, leading family and community education initiatives on dementia, and traveling between offices and care sites as required. Candidates should have substantial dementia/cognitive impairment experience, relevant clinical roles in geriatrics/memory care, ongoing or willingness to pursue dementia certification, and the ability to work flexible hours with reliable private transportation. Required Qualifications
- Minimum of five (5) years of experience working with individuals with dementia or cognitive impairments
- Experience in roles such as Activities Director, CNA, MA, RN/LPN, or other direct care professionals in geriatrics or memory care settings
- If licensed, license must be in good standing
- Actively enrolled in or willing to enroll in a Dementia certification course
- Dementia Care certification preferred or willingness to pursue certification
- Familiarity with behavioral health medications and interventions for this population
- Ability to read and communicate effectively
- Strong written and verbal skills
- Basic computer knowledge
- Ability to manage and demonstrate effective leadership skills
- Must be able to work flexible hours and travel between offices, facilities, etc.
- Must be a licensed driver with an automobile that is insured in accordance with state and/or organizational requirements and is in good working order
