Assess, plan, implement, and evaluate patient care under RN direction, contributing to health status assessments, care plans, and intervention evaluations. Provide basic patient care activities, assist with patient transport, and document all cares provided in the healthcare consumer's record. Communicate findings to the multidisciplinary team and observe responses to interventions. Work three 12-hour shifts weekly from 6:30 PM to 7 AM, including every third weekend and rotating holidays.
