Job Summary
Transitions At Home CNA/Caregiver roles in a Southern Wisconsin Independent Living Facility. Provide personal cares, transfer assistance, household services, and companionship to seniors; assist with mobility, meal support, light housekeeping, and engaging activities; report changes in condition to the RN Case Manager under the RN Clinical Manager’s direction; full-time and part-time schedules (7AM-7PM with open availability, Monday–Friday) with competitive wages and benefits. Requires Medicare Conditions of Participation for Home Health Aide training and a current Wisconsin CNA license; must demonstrate compassion, empathy, and maturity, with the ability to read/write English and communicate effectively.
Required Qualifications
- Must meet Medicare Conditions of Participation for Home Health Aide training program and competency.
- Must have a current Wisconsin CNA License.
- Demonstrate compassion and empathy in caring for individuals in need of support and assistance.
- Ability to carry out directions, read and write in English.
- Strong communication skills.
- Maturity and ability to deal effectively with the demands of the job.
