Job Summary
CNA provides direct patient care under RN supervision, assists with transfers and ambulation, reports changes, maintains infection control and a clean safe home environment, and supports care coordination and documentation per agency policy. Responsibilities include coordinating with Clinicians (HHA/RN/LVN) on patient care and updates, managing biweekly supply orders, collecting HHA timesheets and notes for RNs before IDT meetings, and performing other duties assigned by management; requires reading/writing in English, ability to travel locally, and adherence to agency policies.
Required Qualifications
- High School Diploma preferred or as specified in Medicare regulations §418.76(b)(c)
- Certified Nursing Assistant or Certified Home Health Aide Program or state licensure program meeting §418.76(b)(c)
- Current Driver’s License
- At least one year experience preferred as hospice aide or nursing assistant in hospital/nursing facility/home health/hospice/long-term care facility or completion of CNA/HHA program
- English reading/writing and following instructions in English
- Pass Aide Competency Evaluation Skills Checklist meeting §418.76(c) and written skills test
- Reliable transportation and auto liability insurance
- Criminal history check clearance
