Job Summary
PRN Hospice CNA providing personal care and patient support on Crisis Care team. Responsibilities include baths, hygiene, dressing/undressing, assisting with meals and feeding, shopping for nutritious meals, skin care to prevent breakdown, and reporting significant changes to the Case Manager. Shifts are 12-hour nights (7PM-7AM), with territory in Algonquin, Crystal Lake, Lake in the Hills and surrounding area. Requires state CNA certification and registry, and a valid driver’s license. Offers flexible hours, mileage reimbursement, 401k + match, and a supportive team culture.
Required Qualifications
- Certified by the state as a nurse aide
- List on the state aide registry with a clear record
- 18 years of age unless state law dictates otherwise
- Valid driver’s license with an automobile that is insured in accordance with state requirements
