Job Summary
Document patient vitals, assist with mobility, bathing, and grooming, and maintain accurate records per physician orders. Perform range of motion exercises, change linens, and run errands or prepare meals as directed by the client. Communicate significant status changes to the RN or case manager and attend multi-disciplinary team meetings when required. Travel frequently to private homes for home health visits, adhering to all agency policies and dress codes.
Required Qualifications
- Graduation from an approved certification/program/accredited school/undergraduate/graduate school or post graduate education
- Current, unencumbered state certification to practice as a Certified Nursing Assistant (CNA)
- Professional experience required per state regulations to perform the job as described
- Successful completion of assigned screening/competency testing with a proficiency score of 80% or greater
- Copy of current TB results or Chest X-Ray
- Copy of current CPR
- Ability to be flexible, organized and have the ability to exercise sound judgement to perform the job duties/responsibilities as described
- Ability to regularly, sit, stand, stop, bend, kneel, walk, balance, crouch, crawl, push, pull and reach with hands and arms and other physical activities as deemed necessary by job duties
- Vision and Hearing capabilities within normal limits, with or without aid
- Ability to frequently lift and or move up to 15 pounds and occasionally lift or move 100 pounds or more if applicable
- Ability to perform the anticipated level of physical assist required to provide safe client care
- Ability to travel frequently to complete home health visits
