Provide routine daily nursing care and services to assigned residents in accordance with their assessment and care plans. Identify residents using wristbands and photos before administering treatments, meals, or other tasks. Document all cares in the EMR and CNA cardex, report changes in resident conditions, and track daily intake and fluid goals. Assist with grooming, bathing, dressing, toileting, and incontinence care while monitoring skin integrity and using mechanical lifts as needed. Prepare residents for meals, serve food trays, and perform after-meal care including oral hygiene and denture cleaning. Monitor alarms, bed rails, and safety equipment; respond to resident calls promptly and check on those unable to call. Transport residents to appointments and activities, provide post-mortem care as instructed, and maintain confidentiality of all protected health information.
