Job Summary
Provide personal care to patients including baths, dressing and undressing, assisting with meals and nutrition, and observe for skin integrity; document and report significant changes to the Case Manager; support mobility and daily living activities as part of compassionate hospice care; work Monday–Friday 12pm–8pm in Lake Bluff/Deerfield, IL, with a state-certified nursing assistant license and a valid driver’s license; join a collaborative team delivering high-quality patient care.
Required Qualifications
- Certified by the state as a nurse aide
- List on the state aide registry with a clear record able to deliver care to patients
- 18 years of age unless state law dictates otherwise
- Valid driver’s license with an automobile that is insured in accordance with state requirements
