Job Summary
Provide direct personal care to terminally ill patients, including assisting with hygiene, grooming, toileting, and safe mobility using walkers or wheelchairs. Support meal preparation, feeding, and light housekeeping while observing and reporting changes in physical or mental condition. Offer emotional and psychological support to patients and families, maintain accurate visit documentation, and attend team meetings for Quality Improvement activities. Practice strict infection control and safety protocols at all times. Requires an active CNA license, current CPR certification, driver's license, and six months of direct patient care experience. Required Qualifications
- Active CNA license in the state of employment
- Current CPR certification
- Driver's license and reliable vehicle with insurance
- Six (6) months of direct patient care experience
- Comfortable working in home settings, hospice facilities, or long-term care environments
- Demonstrated compassion, patience, and ability to handle emotional situations
- Willingness to work independently and adapt to changing patient needs
- This is a safety-sensitive position

Desired Qualifications
- Experience in home health, hospice, palliative care, or oncology
- Prior roles in hospital nursing, ICU, geriatrics, med-surg, ER, telemetry, or nursing homes
