Job Summary
CNA Homecare in Waukesha, WI (Full-Time, Day Shift, 8:00am-4:30pm). Sign-on bonus $3,000. Responsible for patient and family satisfaction by assisting with activities of daily living, light homemaker tasks, following the plan of care, and documenting care in the electronic health record. Includes weekend and holiday rotations. Provides patient- and family-centered care aligned with Agrace’s hospice and palliative care philosophy. Key duties include assisting with ADLs (ambulation, transfers, toileting, bathing, dressing, feeding), basic homemaking (light cleaning, meal prep, laundry), obtaining vital signs, communicating changes in condition, and participating in team meetings. Requires completion of CNA program, current CNA certification, HS diploma or GED, and a valid driver's license with vehicle access. Required Qualifications
- High school diploma or GED, or equivalent combination of education and work experience
- Graduation from an accredited Certified Nursing Assistant program
- Current, unrestricted Nursing Assistant Certification in state of employment
- One-year personal care experience, hospice, or home health, preferred
- Valid driver’s license, auto insurance, and access to a reliable vehicle
- Basic Microsoft Office skills
- Experience using electronic health records
- Ability to maintain confidentiality about patients, families, caregivers, staff, donors, and organizational intellectual property
- Ability to use proper body mechanics, safe patient handling, and standard precautions
