Provide personal care, light housekeeping, and assistance with Activities of Daily Living for participants in homes and at the PACE center while exercising independent judgment. Carry out non-skilled treatments including vital signs, transfers, toileting, bathing, and dressing under RN supervision. Collaborate with the Interdisciplinary Team to contribute to care plan development and ensure continuity of care. Assist with meal preparation, recreational activities, and therapy exercises such as positioning and ambulation. Maintain a clean and safe environment by performing daily cleaning and disinfecting in accordance with CDC guidelines. Document accurately in EMR systems and participate in regular team meetings. Complete required Summit Eldercare competencies annually and perform all duties per policy and procedures.
