Job Summary
CNA-Home Care position under Nurse supervision providing personal care and homemaker services across the PACE Rhode Island program. Responsibilities include following participant care plans, documenting ADL/IADL in AxisCare, assisting with toileting, bathing, grooming, meal prep, light homemaking, and supporting behavioral and emotional needs. Performs in-home care, monitors participant condition, reports changes to the Home Care Department, adheres to safety and PPE protocols, supports restorative activities, participates in care-planning meetings, and works rotating weekends/holidays as required. Requires CPR certification, Rhode Island CNA license, and valid driver’s license with reliable transportation. Education requirement is a High School Diploma or equivalent.
