Job Summary
Provide safe personal care and support for terminally ill patients in the Fort Wayne Area, following written plans of care and agency policies. Perform daily hygiene tasks including bed baths, grooming, toileting, and feeding, while assisting with mobility, rehabilitation activities, and meal preparation. Monitor vital signs, document care electronically, and report significant status changes to the nurse. Manage assigned patients in a timely fashion, maintain clean patient environments, and participate in interdisciplinary care conferences. Work 40 hours weekly Monday through Friday with weekend rotations, driving independently to client homes.
Required Qualifications
- High school diploma or GED
- Completion of 105-hour CNA Certification program
- Current/active CNA License
- Current CPR certification or ability to obtain within the orientation period
- Current motor vehicle operator's license
- Access to an automobile with current automobile insurance coverage
- Ability to drive independently to all patient/family/clients' homes
- Visual, hearing, and mental ability to initiate and comprehend written and verbal communication
- Ability to engage in clear, concise verbal/non-verbal communication
- Ability to climb a minimum of three (3) flights of stairs
- Knowledge of emergency procedures according to agency policy
- Ability to remain calm in occasional emergency situations
- Willingness to accept assignments without regard to disease or ethnic origin
- Dependability in areas such as attendance, tardiness, and flexibility for unit coverage
- Willingness to work scheduled holiday rotation
- Willingness to travel in inclement weather
- Willingness to be cross-trained in other departments within the agency
Desired Qualifications
- Minimum six (6) months (one [1] year preferred) CNA/HHA experience, within the last twenty-four (24) months, in the home health, long-term care, or hospital setting
- RN nursing students who have successfully completed courses with a grade of 'C' or better, who currently possess an active CNA License
