Job Summary
Monitor and record patient conditions, including vital signs, intake, output, and exercise, while assisting with bathing, dressing, grooming, and household tasks like laundry and meal preparation. Support the Plan of Care by collaborating with the multi-disciplinary team, teaching families proper patient handling techniques, and communicating updates via the EMR system. Maintain a safe environment through aseptic practices, adhere to federal and state regulations, and fulfill on-call duties as assigned. Requires a Home Health Aide Certificate, one year of CNA experience, active CPR certification, and valid driver's license.
Required Qualifications
- High School graduate or equivalent
- Home Health Aide Certificate or other similar certification in good standing from an accredited training course that meets the standards established by Health and Human Services
- Minimum of one year (1+) of experience as a home health aide or certified nursing assistant (CNA)
- Strong English language both written and verbal
- Active CPR certification
- Valid driver's license
- Reliable transportation
- Insurance
