Job Summary
Perform personal care including baths, oral hygiene, and dressing as assigned, while providing positive communication and maintaining patient safety with proper equipment use. Take vital signs, monitor skin for tissue breakdown, assist with ambulation and transfers, and administer reminders for self-medication per state regulations. Plan nutritious meals, assist with feeding, manage laundry, and maintain living areas as directed. Report significant condition changes and interpersonal issues to the Case Manager, participate in quality assessment teams, and provide respite for families. Maintain clean, orderly environments and adhere to all documentation and care procedures.
Required Qualifications
- Must be a Certified Nursing Assistant (applicable for all States, additional requirements for California residents, see below)
- CALIFORNIA REGULATION: Must be a CNA in good standing AND must have a current Home Health Aide (HHA) certification in good standing in the State of California
- Must have completed a minimum of 75 hours of classroom and supervised practical training
- Licensed driver with automobile that is insured in accordance with organization/state requirements and is in good working order
- Willingness to submit to a criminal background check prior to hire and annually thereafter
- Current CPR certification
Desired Qualifications
- Preferred to have one (1) year experience in community health/hospice or medical/surgical environments
- An Understanding of hospice philosophy and comfortable providing specialized care to the terminally ill
- Ability to work autonomously with little direct supervision
- Successfully demonstrate tact, patience and good personal hygiene
- Ability to communicate effectively
- Strong organizational skills
- Understanding of plan of care documentation
- Flexible in work hours to ensure the patients receive care as provided in the plan of care
- Ability to empathize with the needs of the ill, injured, frail and impaired
- Comfortable addressing issues of death/dying
- Calm demeanor when entering the patients home
