Job Summary

Perform personal care tasks including baths, oral hygiene, dressing, and feeding patients as assigned. Monitor vital signs, assess skin integrity, and assist with transfers and ambulation per nurse or therapist direction. Document patient conditions, report changes to the Case Manager, and maintain clean living areas while adhering to care procedures. Provide respite for families and support light laundry needs when necessary. This role requires a Certified Home Health Aide with current CPR certification and a licensed driver's license. Candidates must demonstrate tact, patience, and the ability to communicate effectively within a hospice philosophy.

Required Qualifications

  • Must be a Certified Nursing Assistant (applicable for all States, additional requirements for California residents, see below)
  • CALIFORNIA REGULATION: Must be a CNA in good standing AND must have a current Home Health Aide (HHA) certification in good standing in the State of California
  • Must have completed a minimum of 75 hours of classroom and supervised practical training
  • Licensed driver with automobile that is insured in accordance with organization/state requirements and is in good working order
  • Willingness to submit to a criminal background check prior to hire and annually thereafter
  • Current CPR certification
  • Ability to read and follow written instructions
  • An Understanding of hospice philosophy and comfortable providing specialized care to the terminally ill
  • Ability to work autonomously with little direct supervision
  • Successfully demonstrate tact, patience and good personal hygiene
  • Ability to communicate effectively
  • Strong organizational skills
  • Understanding of plan of care documentation
  • Flexible in work hours to ensure the patients receive care as provided in the plan of care
  • Ability to empathize with the needs of the ill, injured, frail and impaired
  • Comfortable addressing issues of death/dying
  • Calm demeanor when entering the patients home

Desired Qualifications

  • Preferred to have one (1) year experience in community health/hospice or medical/surgical environments
