Job Summary
Perform personal care tasks including baths, oral hygiene, and dressing patients as assigned. Provide patient and family with positive communication techniques while maintaining confidentiality and safety needs. Take vital signs, monitor skin for breakdown, and assist with ambulation and transfers per nurse or therapist instructions. Plan nutritious meals, assist with feeding, and aid with bedpan or urinal needs. Report significant condition changes to the Case Manager and participate in quality assessment teams. Maintain clean living areas and provide respite for families when on-site. Adhere to documentation standards and care procedures. Requires current CNA and HHA certification, CPR, licensed driver status, and willingness to submit to background checks. Flexible hours to align with patient care plans.
Required Qualifications
- Must be a Certified Nursing Assistant (applicable for all States, additional requirements for California residents, see below)
- CALIFORNIA REGULATION: Must be a CNA in good standing AND must have a current Home Health Aide (HHA) certification in good standing in the State of California
- Must have completed a minimum of 75 hours of classroom and supervised practical training
- Licensed driver with automobile that is insured in accordance with organization/state requirements and is in good working order
- Willingness to submit to a criminal background check prior to hire and annually thereafter
- Current CPR certification
- Ability to read and follow written instructions
- An Understanding of hospice philosophy and comfortable providing specialized care to the terminally ill
- Ability to work autonomously with little direct supervision
- Successfully demonstrate tact, patience and good personal hygiene
- Ability to communicate effectively
- Strong organizational skills
- Understanding of plan of care documentation
- Flexible in work hours to ensure the patients receive care as provided in the plan of care
- Ability to empathize with the needs of the ill, injured, frail and impaired
- Comfortable addressing issues of death/dying
- Calm demeanor when entering the patients home
Desired Qualifications
- Preferred to have one (1) year experience in community health/hospice or medical/surgical environments
