Job Summary
Provide high-quality, resident-centered care under the supervision of an LPN or RN by assisting with activities of daily living, supporting mobility and safety, and delivering basic restorative nursing services. Observe and report changes in resident conditions while maintaining accurate documentation and supporting individualized care plans in accordance with Minnesota Department of Health standards. Work across various skilled nursing and assisted living communities in Minnesota on a full-time or on-call basis, with mileage reimbursement for qualifying travel. Current registration on the Minnesota Nursing Assistant Registry and willingness to travel are required.
Required Qualifications
- Current registration on the Minnesota Nursing Assistant Registry
- Ability and willingness to travel to various Monarch Healthcare Management communities
- Strong communication and teamwork skills
- Passion for providing compassionate, resident-centered care
- Travel is expected for this role
Desired Qualifications
- Long-term care experience preferred, but not required
