Job Summary
Provide resident-centered nursing care and daily living assistance, including bathing, toileting, grooming, and dressing under RN supervision. Assist with transfers, repositioning, walking, and range of motion exercises while collecting non-invasive specimens and vital signs. Prepare meals, support dining experiences, and maintain clean living areas. Document resident interactions, behaviors, and dietary outcomes; communicate changing conditions to the charge nurse. Participate in wellness programs and departmental committees. Maintain CNA certification, complete annual competency testing, and adhere to infection prevention policies.
Required Qualifications
- Minimum age of 16
- Certified Nursing Assistant (CNA) certification with the State Board of Nursing, or state certified CNA within four months of date into the position by being enrolled in a training program or in the process of obtaining certification through Endorsement
- Satisfactorily completes yearly department specific competency testing
- Maintains a record of continuing education credits for each year
- A minimum of 12 hours continuing education are required annually
- Maintains all department specific required certifications
- Certified and registered with the Unlicensed Assistive Person (UAP) Registry (when applicable, for applicable states)
- Motor vehicle report and proof of valid driver's license (if role classifies as Sanford Category II Driver)
- Medically qualified to perform the essential driving functions of this position
Desired Qualifications
- High school graduate or general equivalency diploma (GED) preferred, but not required
- Prior clinical or nursing assistant experience in long-term care preferred
