Job Summary
Assist patients with bathing, dressing, grooming, toileting, and mobility while monitoring vital signs and documenting care in the EHR. Provide medication reminders, prepare light meals, perform light housekeeping, and offer companionship to help clients maintain independence in their homes. Communicate any changes in patient condition to the supervising nurse. This role requires current Texas CNA and CPR certification, a valid driver's license, and two years of home health experience preferred. Located in San Antonio, TX, with immediate PRN openings available.
Required Qualifications
- Current Texas CNA Certification
- Current CPR certification
- TB test taken within the past year
- High school diploma or equivalent
- Computer skills required
- Reliable transportation
- Valid driver's license
- Strong communication skills
- Compassionate, patient-focused attitude
