Job Summary
Provide personal care and support to hospice patients under the supervision of a Registered Nurse in an interdisciplinary team, performing tasks such as baths, dressing, nutrition, feeding, and monitoring. Maintain patient safety, assist with ambulation and transfers, take and record vital signs, monitor skin integrity, administer reminders for medications, assist with light housekeeping including laundry, and offer respite for family/caregiver. Communicate effectively with patients and families, adhere to care procedures and plan of care documentation, and participate in quality assessment and performance improvement activities. Maintain a calm, respectful, and compassionate demeanor in the patient’s home environment, and demonstrate flexibility in work hours to ensure patient care as outlined in the plan of care.
Required Qualifications
- Must be a Certified Nursing Assistant (CNA)
- California residents: current Home Health Aide (HHA) certification in good standing in CA
- Preferred: 1 year experience in community health/hospice or medical/surgical environments
- Completed a minimum of 75 hours of classroom and supervised practical training
- Licensed driver with automobile insured as required
- Willingness to submit to criminal background check prior to hire and annually thereafter
- Current CPR certification
