Job Summary

Monitor and report changes in individuals' conditions to the LPN, assist with maintaining accurate patient records, and help manage vital signs and medical paperwork. Ensure the health, safety, and wellbeing of individuals by conducting monthly face-to-face checks, monitoring adaptive equipment, and tracking medication cycles and inventory. Support staff in administering non-oral medications, attending medical appointments, and ensuring program compliance with state regulations. Represent Bell professionally while communicating with families, case management, and medical personnel to keep stakeholders informed of health issues. Participate in department meetings, attend required training, and adjust schedules as needed to meet individual needs.

Required Qualifications

  • HS diploma/GED
  • current state-issued certificate
  • two (2) years professional experience
  • proficient understanding of medical terminology
  • strong organizational skills
  • attention to detail
  • ability to complete physical and PPD tests
  • at least 21 years old
  • valid driver's license
  • insurable driving record
  • reliable transportation
  • Current physical and Mantoux (TB) test
  • PA State Police background check
  • Act 33 and/or FBI clearances
