Required Qualifications
- CNA certification
- Licensed to work as a CNA in New York
- Updated Resume
- 2 Legal IDs
- Social Security Card
- New York License & Registration
- Two Reference letters
- School Diploma or Completion Certificate
- Malpractice Insurance
- Physical Exam and PPD (current: within 10 months)
- Chest X-ray and TB Questionnaire, if PPD is positive
- Proof of immunity for Mumps, Measles, Rubella and Varicella (current: within 10 months)
- Drug Screen (current: within 2 months)
- Flu Shot (during flu season, September to May)
- COVID-19 Vaccination Card
