Job Summary
Coordinate direct and indirect patient care under the Clinical Nurse Supervisor, managing rooming, tracking, and self-management coaching for clinic patients. Develop care plans through collaboration with providers and families, facilitating access to specialty resources and managing test results and medication adherence. Maintain accurate electronic health records, ensure HIPAA confidentiality, and support quality improvement activities while managing supplies and equipment. Assist with telephone inquiries and appointment scheduling, utilizing evidence-based practices to address unmet health and social needs.
Required Qualifications
- Current licensure as an LPN or CMA in the State of Idaho
- Must be able to pass any and all background checks required by law
- 1 years of CMA or related experience
- Ability to deal tactfully and effectively with patients (and their families), coworkers, hospital staff and physician
- Add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals
- Compute rate, ratio, and percent and to draw and interpret bar graphs
- Effective organizational skills and demonstrates ability to maintain accurate notes and records
- Apply commonsense understanding to carry out instructions furnished in written, oral or diagram form and deal with problems involving several concrete variable in standardized situations
- Must frequently lift and/or move up to 50 pounds
- Must occasionally lift and/or move more than 50 pounds
- Specific vision abilities required by this job include close vision, distance vision, color vision, and depth perception
Desired Qualifications
- Bilingual (English and Spanish)
- 3-5 years' experience in clinical or community health settings preferred
- Previous experience with health IT systems and data reports preferred
