CM Social Worker MSW - PRN, Weekends

Develop comprehensive transitional care plans for patients with complex psychosocial needs in Inpatient and Emergency Department environments while promoting patient and family wellness and improved care outcomes. Collaborate with physicians, service team members, external agencies, and families to ensure access to appropriate hospital and community resources. Manage and support effective transitions planning to enhance quality of care across the care continuum. This role is available on a PRN basis for weekends at Piedmont Walton Hospital.
