Clinician Support Specialist Tele Triage, PT Remote

Conduct daily tele-triage support by preparing clinical documentation, entering lab and imaging results, and performing medication reconciliation for acute visits. Complete basic opening of encounter notes, document triage encounters for CCM services, and send updates to primary care providers. Make phone calls to facilities to gather information and check communication channels multiple times daily to ensure patient needs are met. Inform patients and families of recent visits and new orders, deferring to providers when necessary. Communicate care transitions to the Tele Triage team to enhance patient education. Work remotely Monday through Friday from 10 am to 2 pm.
