Job Summary
Clinician Support Specialist provides field-based support to clinical providers, ensuring providers can focus on patient care. Responsibilities include scheduling coordination, documenting encounters, medication reconciliation, hosting telehealth sessions, facility coordination and patient-family communications, and assisting with discharge planning and triage as needed. Travel up to 60 miles from home to facilities, with on-site support in skilled nursing and assisted living settings. Requires HIPAA-compliant practices, ability to work with multiple care teams, and strong documentation skills. Mon-Fri, 8-hour dayshift; fuel card provided.
Required Qualifications
- High School Diploma or equivalency
- Valid driver’s license and reliable transportation
- Expected to travel up to 1 hour from home address
- Typing speed of 45wpm or greater
- Medical Scribing experience preferred
- Certified Clinician Support Specialist degree a plus
