Job Summary
Conduct strengths-based, trauma-informed therapy and targeted case management services for agency-approved clientele, including direct mental health support, family assessments, and home visits to ensure safety and stability. Develop individualized treatment plans, monitor client progress, and coordinate with interdisciplinary teams to achieve timely permanency goals. Maintain accurate documentation in the electronic health record and UC portal, provide crisis intervention as needed, and facilitate referrals for mental health, medical, and educational services. Ensure culturally sensitive care and appropriate resource linkage while upholding client confidentiality.
Required Qualifications
- Must be at least 21 years of age
- Master's degree in social work, clinical psychology, or related relevant behavioral science degree in which direct clinical experience is a program requirement
- Registration required by the California Board of Behavioral Sciences (BBS) as an Associate Clinical Social Worker (ASW) or Associate Marriage Family Therapist (AMFT) or Associate Professional Counselor (APCC)
- Individual Malpractice Insurance
- Proficiency in general office software including MS Word, MS Excel, and MS Outlook
