Job Summary
Behavioral Health Clinician conducts comprehensive bio-psychosocial assessments and develops care plans; provides direct patient care including case management, care coordination, crisis intervention/safety planning, counseling, and emotional support to patients and families; performs referrals and discharge planning; maintains timely documentation per licensing and third-party requirements; floats to all Natchaug sites (Danielson, Dayville, Enfield, Groton, Mansfield, Norwich, Old Saybrook, Vernon) based on program needs; participates in inter-agency collaboration with school social workers, principals, DCF, and court systems; eligible for a sign-on bonus up to $2,500; requires master’s degree and current CT licensure/certification as LMSW, LPC-A, LMFT-A, or CADC; valid driver’s license; benefits package details provided by Hartford HealthCare.
Required Qualifications
- Master’s degree in social work, or counseling, or marriage and family therapy or related human service field
- Current Connecticut LMSW, LPC-A, LMFT-A, or CADC
- Valid Driver's license
- Excellent communication skills
- Cultural competence and awareness of health disparities
