Job Summary
Provides comprehensive bio-psychosocial assessments and develops care plans in collaboration with interdisciplinary teams. Delivers evidence-based clinical services, utilizes specific therapeutic methods, monitors progress, and revises plans to meet patient/family needs. Performs direct patient care including case management, care coordination, crisis intervention/safety planning, counseling, and discharge planning; supports education to patients/families and ensures timely patient-centered documentation compliant with licensing and third-party requirements. Participates in administrative and development activities, contributes to hospital/unit programming, and supports cost-effective resource utilization.
Required Qualifications
- Master's Degree in Social Work, Counseling, Marriage and Family Therapy or Related Human Service Field
- LMSW, LPC-A, LMFT-A, or CADC
- Valid Driver's License
- Computer proficient
- Ability to assess problems, determine solutions and make decisions within a timely manner
- Utilize supervision and team approaches in the context of patient assessment and clinical interventions
- Excellent communication skills, with the ability to establish relationships within and outside their entity
- Demonstrates competencies, including knowledge and behaviors, to meet age specific patient needs
- Focused on personal, patient/client and unit/staff safety
- Self-directed in maintaining currency of validations, certifications, licensure and training
- Cultural competence and sensitivity to health disparities-inequalities and injustices
- Develops and maintains cultural competence with an understanding of the history, traditions, values, and family systems of client groups as relates to health care and decision-making
