Job Summary
Conduct psychosocial assessments, treatment planning, and discharge planning under supervision while providing counseling to clients requiring complex intervention. Perform or direct patient screening, intake processes, and referrals to community services, ensuring accurate assembly of medical, psychological, and financial information. Monitor client progress through interviews and chart reviews, writing concise progress notes and acting as a client advocate to coordinate required services or resolve crises. Develop community contacts, participate in strategy sessions, and represent the facility in community meetings. Provide patient and family education during admission, hospitalization, and discharge, and counsel individuals and groups to help clients overcome dependencies and adjust to life.
Required Qualifications
- Masters degree in a related area from an accredited college or university
- eligibility to be licensed in the state of Georgia
- associate licensure in the state of Georgia
- full clinical licensure in the state of Georgia
