Job Summary
Conduct home and community-based assessments, planning, and treatment for families enrolled in Family Preservation services to prevent out-of-home placement. Deliver six to nine months of intensive services through direct work with individuals, groups, and families, including regular treatment plan meetings and service reviews. Act as a liaison with collateral resources such as referring agencies, schools, family court, and other personnel while making clinical decisions within a team structure with a Clinical Case Manager. Complete all records and client information adhering to agency, county, and state policies. Report to the Program Coordinator with a mix of on-site and remote duties, including travel to client homes. Requires a Master's degree and one year of post-graduate experience.
Required Qualifications
- Masters degree in Social Work, Mental Health Counseling or related field
- Minimum of one year of post-graduate experience
- Must have a valid drivers license

- License Preferred: LMSW, LCSW, LMHC, or LMFT
