Job Summary
Provide intake, assessment, and treatment planning for clients in outpatient and home settings while managing case services and community resources. Monitor client placements, discharge planning, and step-down services, maintaining accurate records with full compliance and proficiency in the agency's Electronic Health Record. Collaborate with community providers and health professionals, adapting to change and taking ownership of problem resolutions. Requires evening and on-call hours; competitive pay of $60,000 annually with comprehensive benefits including medical, dental, vision, tuition reimbursement, and a 403(b) match.
Required Qualifications
- Graduate degree in social work, psychology, or a closely related field
- Demonstrated computer proficiency and experience with electronic health records
- Colorado driver's license and access to transportation during work hours
- Must already be authorized to work in the US; sponsorships not available
- Requires some evening and on-call hours
Desired Qualifications
- Colorado professional license (i.e., LPC, LCSW, LMFT) or license eligible
- At least one year of mental healthcare experience treating children/adolescents
- Previous experience working in a home-based program
- Spanish language skills
