Clinician/Care Manager (Union) 26-49

Conduct intensive therapy and case management for individuals and families across outpatient, home-based, and residential settings. Provide individual, family, and group therapy while developing materials for group rotations. Complete intake assessments, coordinate professional evaluations, and link clients with community resources for housing, medical care, and employment. Monitor person-centered plans of service, ensuring revisions reflect individual preferences and facilitating implementation across school, employment, and residential environments. Serve as primary advocate and liaison with families, guardians, and agencies. Participate in evidence-based practices such as TF-CBT, ACT, and IDDT, and coordinate with medication-assisted treatment programs. Perform after-hours on-call participation and crisis intervention during the workday. Complete OBRA and ADOS-2 assessments as directed, develop behavioral plans, and visit residential placements monthly to monitor progress.
