Clinical Triage Specialist RN or LPN, Eves, Nights, Weekends - PA/NJ residents only

Job Summary
Answer telephones, prioritize clinical triage calls, and coordinate services using approved nursing protocols to advise patients on self-care or appropriate care levels. Verify patient demographics, enter updated information into the electronic health record, and document symptoms, assessments, and advice provided. Serve as an escalation point for clinical patient issues, manage escalated electronic messages, and independently resolve patient concerns or schedule appointments within the role's scope. Partner with Access Center teams and practice clinical staff to assist with medication refills or referrals. Maintain productivity, schedule adherence, and quality standards while working nights and weekends.
Required Qualifications
- Graduate of an accredited nursing program
- Active Registered Nurse licensure in the state of Pennsylvania and New Jersey or other nursing compact state and other states as deemed necessary by state law
- Active LPN licensure in the state of Pennsylvania and New Jersey or other nursing compact state and other states as deemed necessary by state law
- Minimum 2 years recent clinical experience in a physician office, home health, critical care and/or emergency room
