Clinical Triage Specialist (LPN/RN) - Primary Care; Part-Time, 16 hours/week (PA & NJ Residents Only)

Job Summary
Answer telephones, prioritize clinical triage calls, and coordinate services using approved nursing protocols and guidelines. Verify patient demographics, enter updated information into the electronic health record, and document symptoms, assessments, and advice provided. Serve as an escalation point for clinical patient issues, manage escalated electronic messages, and independently resolve patient concerns within the role's scope. Schedule appointments, refer calls to appropriate medical facilities, and partner with Access Center teams and practice clinical teams regarding medication refills or clinical concerns. Consistently meet productivity, schedule adherence, and quality standards. Requires active RN licensure in Pennsylvania and New Jersey with minimum two years of recent clinical experience.
Required Qualifications
- Graduate of an accredited nursing program
- Active Registered Nurse licensure in the state of Pennsylvania and New Jersey or other nursing compact state and other states as deemed necessary by state law
- Minimum 2 years recent clinical experience in a physician office, home health, critical care and/or emergency room
